A Case of Native Hip Pseudomonas aeruginosa Septic Arthritis Caused by Vesico-acetabular Fistula

A 61-year-old man with a past medical history significant for metastatic rectal cancer treated local resection, chemotherapy, and radiation complicated by vesicocutaneous fistula presented subacute ambulatory dysfunction secondary to right hip pain. Imaging studies were consistent effusion, fluoroscopyguided aspiration revealed septic arthritis caused Pseudomonas aeruginosa. Resistance patterns identified this strain of be the same one that pyelonephritis was during hospitalization. Although further diagnostic imaging not pursued, presentation most vesico-acetabular causing native arthritis. This is very rare complication therapy serves as reminder keep broad differential atypical presentations in patients who have undergone extensive radiation.
